Submit this form to request permanent deletion of your ClinicOS account and associated data.

What happens next:

  • We'll verify the request from the email tied to your account.
  • Your account, patient records, photos, simulations, and voice notes will be permanently deleted within 30 days.
  • Limited records may be retained where required by law (e.g. financial records, audit logs) for up to 7 years.
  • You'll receive a confirmation email once deletion is complete.
